Class II subdivision malocclusion features are the presence of Class I molar relationship on one side and Class II molar relationship on the other side due to the distal positioning of the mandibular first molar in relation to the maxillary first molar, with maxillary or mandibular dental midline deviation. Its treatment includes either four premolar extractions or asymmetric extractions comprising two premolar extractions (mandibular and maxillary) at the Class I side and one premolar extraction (maxillary) at the Class II side. The aim of this case report was to show the treatment of a Class II subdivision malocclusion by means of asymmetric extractions. Such approach enabled the use of a more predictable orthodontic mechanics with little patient’s compliance. Results were satisfactory showing an excellent occlusion, with correction of the mandibular midline and the canine relationship in the initial Class II side, and good facial aesthetics.
