Introduction: The Class II subdivision constitutes a malocclusion difficult to treat considering the numerous therapeutic possibilities. Considering that this malocclusion usually presents a dentoalveolar involvement, depending on patient’s age, anteroposterior occlusal discrepancy, crowding and facial profile convexity, the best result can be achieved by means of the asymmetric extractions protocol. Objective: This study aimed at showing the asymmetric extractions protocol treatment of a female patient who presented with a Class II subdivision malocclusion and presence of a maxillary supernumerary tooth. Results: The outcome was satisfactory, with correction of the dental midlines deviation and anteroposterior discrepancy. Conclusion: The Class II treatment with asymmetric extractions produces efficiency in the discrepancy correction as well as smile aesthetic and face harmony as treatment outcome.
