Multidisciplinary approach in the treatment of root dilacerations: case report

Root dilaceration, or abnormal curvature in the root, prevents tooth eruption and is an anomaly that can be caused by different etiological factors, such as dental trauma in childhood. In this paper, we report a case of a patient with root dilaceration of the impacted right upper central incisor, with severity of the degree of dilaceration and coronary inclination. The evaluation of the degree of severity of root laceration as well as of dental inclination was made by means of angular measurements of the long dental axis with root inclination (α angle) and radius (r), and the angle of the long dental axis with the APo line (ß angle). At the end of the orthodontic therapy, the patient underwent an endodontic treatment to treat a pulp necrosis generated by orthodontic movement. The prominent apical third was surgically removed. At the end of the treatment, we recorded a good triaxial position of the remaining root, which presented surrounding bone density on the mesial, distal, and palatal surfaces, proving that there was no bone involvement, a determining factor in the final stabilization. The therapeutic success in this clinical case was possible firstly by the multidisciplinary interaction involving Orthodontics, Endodontics and Surgery, favoring the non-extraction of the tooth #11, preserving the esthetics and function. Finally, we suggest that long-term root stability be monitored.
