Introduction: Orthognathic surgery is a treatment modality focused on the correction of dentofacial deformities. It requires a multidisciplinary team to be conducted, by means of the interaction between oral and maxillofacial surgeon and orthodontist. The association of orthodontic treatment with surgical techniques, such as bilateral sagittal split osteotomy and Le Fort I osteotomy, has brought great achievements in terms of correction of skeletal malocclusions with the final objective of a functional occlusion, aiming at the health of orofacial structures, facial and dental esthetic harmony, and stability of results. This paper presents a clinical case of Class III dentofacial deformity depicting contemporary orthosurgical management. Case report: 23-year-old male patient diagnosed with skeletal Class III malocclusion, presenting history of previous compensatory orthodontic treatment. Orthosurgical correction was indicated with the objective of improving masticatory, respiratory and phonatory functions, combined with better smile and facial esthetics. Results: After 16 months of orthodontic preparation, orthognathic surgery was conducted for maxillary advancement, mandibular setback and genioplasty. In 6 months of post-operative control, with finished orthodontic treatment, the patient showed favorable functional and esthetic results. Conclusions: The accurate indication of orthosurgical management represents an effective method for treatment of skeletal malocclusions, affording satisfactory functional results, with adequate predictability and stability.
