Introduction: The planning in orthognathic surgery has evolved in the past years in order to improve the treatment possibilities and predictability of results. Objective: The aim of this study is to show a new method for virtual surgical planning with a natural head position transfer technique from photograph register to the computer tomography soft tissues images to orientate the composite skull. Methods: a photograph register method and 3D image acquisition is described, showing how the natural head position is transposed between them. The composite skull of this planning was obtained from multi-slice computerized tomography and intrabucal scanning. The composite skull orientation was made by means of elected anatomical points, equivalent in both images. Conclusion: It an be concluded that this is a valid and secure method, based on the reliability of the natural head position to clinical analysis and planning.
