Orthodontic-surgical treatment of unilateral cleft lip and palate associated with complete crossbite and absence of maxillary lateral incisor and premolar: case report

Introduction: Patients with cleft lip and palate require primary surgeries in the early years of life as the first part of treatment. In addition to these surgeries, in some cases orthognathic surgery is also necessary after orthodontic preparation in the permanent dentition, as a form of correction of sagittal discrepancy, due to deficiency of maxillary development in the sagittal direction. Case report: In this context, this article reports the case of a 9-year-old female patient with unilateral cleft lip and palate in the right side, skeletal Class III, complete crossbite, absence of maxillary right lateral incisor and second premolar, and negative discrepancy in both arches. The patient was treated with rapid maxillary expansion, maxillary protraction, secondary bone graft and fixed orthodontic treatment, with maxillary left lateral incisor and second premolar extraction, followed by orthognathic surgery to maxillary advancement. Result: At the end of the treatment, a good sagittal and transverse occlusion was obtained and an important aesthetic gain was achieved for the patient.
