Treatment of vertical Class II with mini-implant in the infrazygomatic crest of maxilla

Introduction: Class II treatment with vertical growth pattern has always been a great challenge for the orthodontist. In recent years, the intrusive mechanics anchored in mini-plates or mini-implants has been gaining expression and becoming increasingly popular because of the practicality and efficiency of the skeletal anchor- age. Description: Vertical Class II is marked by the involvement of the maxilla, which presents excessive vertical growth, and displaces the mandible downwards, inducing it to an clockwise rotation, capable of creating dental and skeletal Class II relationship. In this article, we will discuss the use of a mini-implant inserted in the infra-zygomatic crest for treatment of a vertical Class II patient, through the intrusion of the upper molar region. Results: This extra-alveolar technique, allowing dental intrusion, causes mandibular rotation in counterclockwise direction, as it eliminates the posterior fulcrum. Conclusions: The technique is quite effective because it allows to achieve satis- factory results with minimal patient collaboration.
