Objective: This study aims to present a case report of an adult female patient with severe maxillary constriction treated with miniscrew-assisted rapid palatal expansion. Case report: Female patient, 26 years of age, presented to treatment with severe maxillary constriction and posterior crossbite. The patient presented an advanced midpalatal suture maturation stage. Miniscrew-assisted rapid palatal expansion (MARPE) was indicated as treatment option. The MARPE expander used four miniscrews positioned parasuturally after the third palatal rugae. Bone perforations were performed in the palatal posterior region before the expander placement. The patient was oriented to activate the screw 1/4 turn twice a day. Results: After 8 days, the interincisal diastema opening was observed. The correction of the maxillary constriction and posterior crossbite was achieved after 20 days of screw activation. Conclusion: MARPE associated with bone perforations promoted a successfully opening of the midpalatal suture. MARPE was an effective option to treat the severe maxillary constriction in an adult patient with advanced midpalatal suture maturation.
