Introduction: The association of auxiliaries, such as mini-implants, with aligners allows to expand the range of therapeutic options, especially for complex cases in which anchorage is a critical point. In addition, treating a Class II malocclusion by associating extra-alveolar mini-implants with aligners may favor the achievement of more accurate, predictable and faster results that would hardly be achieved with the isolated use of aligners. Objectives: The present paper describes the biomechanics of the combined use of extra-alveolar mini-implants and aligners in a patient with Class II dentoalveolar malocclusion. Conclusion: The association of extra-alveolar mini-implants and aligners allows greater biomechanical efficiency in the resolution of Class II malocclusion. Extra-alveolar mini-implants connected by means of elastic chain directly to buttons bonded to the teeth become an excellent option for anchorage reinforcement, without the need for patient cooperation with the use of Class II intermaxillary elastics.
