Interceptive treatment of severe Class II malocclusion with Thurow appliance

Introduction: Thurow maxillary splint, or Thurow appliance, is a maxillary splint associated to a high traction, effective in Angle Class II Division 1 correction, resulting in both anteroposterior and vertical changes. Objective: This case report reviews Thurow appliance clinical application, describing its skeletal and dentoalveolar effects in the correction of a severe maxillary protrusion. Case report: The patient was in prepubertal growth spurt, with premolars still in eruption process, and was diagnosed with Class II skeletal and dental malocclusion, deep overbite and an excessive overjet of 14mm. Results: Distalization and intrusion of upper teeth was successfully promoted, as well as restraining maxillary vertical growth, improving gingival display, reducing anterior trauma risk and allowing reduction of mandibular plane angle by its counter-clockwise rotation. Conclusions: This therapeutic alternative may be of great functional and esthetic worth when well indicated, especially during growth spurt.
