Introduction: Class II, division 1, malocclusion has several etiologies, and its clinical and radiographic characteristics are aggravated with growth. Objective: To show a clinical approach to Class II skeletal malocclusion treatment using the Thurow appliance, highlighting its influence on bone growth. Description: Female patient, aged 8 years and 9 months, Class II division 1 molar occlusion, protruded maxillary incisors, 100% overbite and accentuated overjet. The cephalometric analysis showed that the maxilla was protruded in relation to the anterior skull base, while the mandible was retracted and had a Class II skeletal pattern. The treatment option was to install the Thurow appliance. Class II molar relationship and marked overjet correction were observed. In the final cephalometric analysis, the maxilla was well positioned in relation to the base of the skull, and mandible was still retracted. In the anterior region, the maxillary incisors were uprighted and retracted, while the mandibular incisors kept their initial position. Conclusion: The Thurow appliance acted by controlling maxillary growth and improving the relationship between the arches. In addition, it corrected the molar relationship and decreased the protrusion of the maxillary incisors.
