Introduction: Contemporary Orthodontics is char- acterized by the high demand of adult patients, and transdisciplinary and integrated approaches are becoming increasingly common. This new trend has given rise to new orthodontic diagnostic protocols, based on cone beam computed tomography (CBCT), allowing the realization of integrated plans aimed at the complete oral rehabilitation of the patient. With regard to orthodontic mechanics, skeletal anchorage is one of the items that deserves special attention, and mini-plates stand out among devices with such purpose, as they better support loads during orthodontic movement, in addition to providing better three-dimensional control. Objective: The purpose of this article is to describe a case report, with a multidisciplinary approach, in which the diagnosis was based on a tomographic study and the treatment plan was conducted with skeletal anchorage with miniplates. Results: The orthodontic treatment recovered the space for the tooth #35 implant, corrected the maxillary and mandibular anterior crowding without the need for extraction of premolars and interdental wear, achieving a functional and balanced occlusion. It was also possible to observe the improvement of the aesthetic of the smile, with good exposure of the maxillary incisors, proportion of the teeth and improvement in the relation of the lips. Conclusions: Orthodontics has taken a leading role in this case of oral rehabilitation due to 3D diagnosis and skeletal anchorage by means of miniplates, thereby enabling aesthetic rehabilitation treatment at a lower biological cost.
