Introduction: Despite affecting a small portion of the general population, the treatment of Class III malocclusion represents a major challenge for orthodontists. There are several alternatives for Class III correction in adult patients, such as transparent aligners and fixed appliances, associated with mini-implants or intermaxillary elastics. Objective: This article aims to describe the treatment of a patient with high aesthetic demand, who presented asymmetric Class III malocclusion, by means of ClearCorrect aligners, associated with unilateral Class III elastics as anchorage. Additionally, data were collected regarding symptoms and patient satisfaction with treatment. Results: At the end of treatment, which lasted 10 months, a Class I canine relationship was established, the correction of the lower midline and adequate dental alignment and leveling were achieved. The patient reported high satisfaction with the device’s aesthetics and the results obtained. Final considerations: There was a considerable improvement in the asymmetric Class III malocclusion, and Class III elastics were effective in ensuring anchorage during the distalization of the mandibular posterior teeth on the left side. The proposed treatment fulfilled the patient’s expectations regarding time and aesthetics.
