Treatment of skeletal Class III malocclusion with premolar extraction

Objective: This case report aimed to present the treatment of a patient with Class III malocclusion and forward position of the upper and lower anterior teeth. This malocclusion resulted in a forward position of the lower lip, with a negative impact on the facial profile, as the absence of passive lip sealing. The treatment was performed using a fixed metallic appliance, with the extraction of the upper and lower first premolars, followed by retraction of the anterior teeth. Conclusion: It was observed that the treatment was able to improve facial, occlusal, and functional aspects. However, anatomical limitations proved to be limiting factors to achieve passive lip sealing.
