Class III treatment, using extra-alveolar miniscrews in the buccal shelf associated with self-ligating passive appliances with individualized prescription: case report

Introduction: Angle Class III malocclusions of moderate dentoalveolar or skeletal nature may be treated by extractions and distalization of the mandibular arch and/or mesialization of the maxillary arch. To achieve the desired final dental relationship, intermaxillary devices or several types of conventional anchorage resources can be used; however, all of them promote some kind of side effect, which can be undesirable and difficult to control. The introduction of buccal shelf extra-alveolar miniscrews has increased the efficiency of lower arch distalization, simplifying orthodontic mechanics and controlling unwanted side effects through a minimally invasive procedure. The greatest advantage of this device is that the body of the miniscrews is positioned far from the roots of the teeth to be moved, allowing greater sagittal movements, without the need to reposition these devices or use cursors. Furthermore, another advantage to be highlighted is the simplicity of installation, with good patient compliance. Objective: To report a clinical case in which buccal shelf extra-alveolar miniscrews were used as an anchorage resource to perform lower arch distalization, associated with passive self-ligating appliances with individualized prescription, in the correction of a mild skeletal Class III with an increased anteroinferior facial height. Conclusion: The association of extra-alveolar miniscrews with the passive self-ligating appliance system proved to be efficient in the correction of Class III malocclusion; it simplified orthodontic mechanics; inhibited unwanted side effects in the upper arch; achieved excellent torque control; reduced lower incisor inclination; distalized the lower molars with an intrusion component; promoted a smooth counterclockwise rotation of the mandible; decreased anteroinferior facial height; and reduced the need for patient compliance.
