Objective: To report the clinical case of a patient with skeletal and dental Class II malocclusion, and anterior open bite. Case report: After diagnosis and clinical and facial assessment, an orthodontic-surgical treatment was chosen. An orthodontic appliance was installed, and the maxillary second premolars (#15 and #25), left mandibular first premolar (#34), and right mandibular first molar (#46) were extracted. An orthodontic mini-implant was used for inferior retraction. After presurgical decompensation, the patient was referred for maxillary impaction surgery, mandibular advancement, and mentoplasty. After the surgical stage, the patient returned to complete her orthodontic treatment. Conclusion: The results obtained at the end of the treatment were good dental occlusion, with disocclusion guides and improvement in facial profile and contours.
