Class II treatment in hyperdivergent patient with maxillary intrusion, using self-ligating passive appliances with individual prescription, associated with orthodontic miniscrews

Introduction: Correction of the Class II malocclusion in growing hyperdivergent patients requires a different approach from the traditional orthodontic treatments, which do not present good results. Objective: This article presents, by means of a case report, a suggestion of protocol to correct this type of dentofacial deformity, with the containment of the vertical growth of the maxilla. The maxillary intrusion of this growing patient was performed using two miniscrews in the posterior region of the palate, associated with a transpalatal bar bonded to the crowns of the molars and premolars, together with two miniscrews in the anterior region. Results: This approach intruded the maxilla, promoting a counterclockwise rotation of 3.0° in the occlusal plane (PHF.Ocl Pl from 11.5° to 8.5°). This mechanics also promoted a reduction of 5.5° in the mandibular plane (FMA from 31° to 25.5°) and of 1.5° in the ANB (ANB from 6.0° to 4.5°). The dental compensations were attenuated with individualized prescriptions for self-ligating brackets, with the aim of enhancing the orthopedic correction. The inclination of the maxillary incisors increased by 3° and of the mandibular incisors increased by 2.5° (1.Pp from 111° to 114° and IMPA from 96.5° to 99°). The prognosis of this treatment depends on the amount of maxillary intrusion, associated with the growth potential of the mandible.
