Treatment of Class II division 1 malocclusion with removable functional orthopedics and ixed orthodontics: A case report with a 5-year follow-up

Introduction / The association of orthopedic and ixed orthodontic appliances has proved an effective therapeutic option in the treatment of Class II division 1 malocclusion in growing patients. However, skeletal, dental and soft-tissue changes can only be considered as satisfactory with long-term treatment stability. / Objective / A two-phase orthopedic/ orthodontic treatment of a patient with Class II division 1 malocclusion and on craniofacial growth and development, is presented herein. / Methods / In the irst phase, an Activator removable functional appliance was used for a year, followed by ixed orthodontic therapy concomitantly with Activator used as active retention. The inal phase was supplemented with the use of Class II elastics, and lasted 2 years and 5 months. Total orthopedic/orthodontic treatment time was 3 years and 5 months. / Results / Results were satisfactory with overbite, overjet and molar relationship correction, as well as achievement of passive lip seal and a harmonic tending towards a straight proile. Follow-up carried out 5 years after the ixed appliance had been removed revealed maintenance of orthodontic and facial outcomes. Time control is important, since about half of total relapse occurs within the irst two years after treatment completion.
