Safe limits for nonsurgical maxillary expansion in adult patients

Controversies regarding the correction of transverse problems by means of nonsurgical maxillary expansion in adult patients are many and some of the main doubts are: When to indicate nonsurgical maxillary expansion? Which maxillary expansion protocol should be used? The procedure is safe? What are the advantages and disadvantages for the patient? The lack of scientific basis justifying treatment protocols for nonsurgical maxillary expansion in adult patients, in the short and long term, can lead even the most expert orthodontists to skepticism. Objective: This case report presents the maxillary atresia treatment in a 32-year-old patient by means of the nonsurgical rapid maxillary expansion, using the Hyrax appliance and fixed orthodontics, associated with the use of Class II elastics. Conclusion: The success of the proposed therapy was achieved in the reported case. However, the success of this therapy is dependent on the following factors: the existence of a relative maxillary atresia, the severity of the initial atresia, the non-complete closing of intermaxillary suture, the absence of gingival recessions and the possibility of performing a dentoalveolar compensation with minimal or null skeletal effect.
