This paper is a continuation of a series of articles regarding the diagnosis and treatment of the eruptive alterations of the permanent incisors. Here, a new aspect of the problem is discussed: the irregularities in the eruption of the upper anterior teeth due to or associated with a radiolucent area like cyst, diagnosed during the first transitional period of the mixed dentition. Also, a root dilaceration of the upper central permanent incisor and an ectopic eruption were diagnosed, which led to a real dental transposition between the upper right lateral incisor and upper right canine. Orthodontic traction of the central incisor was performed after removal of the cyst, and the transposition was kept during the orthodontic treatment. The final details of the occlusion were achieved with cosmetic dentistry, through the reconstruction of the upper canine and lateral incisor. Both specialities worked together aiming to esthetically restore the upper anterior area.
