The adaptability of the periosteum makes it possible to understand the clinical results of some orthodontic procedures such as R.E.M. and the mandibular anterior projection by functional appliances. Orthopedic appliances estimulate the periosteum formation by mecanotransduction of forces, inducting the periosteum to react deposing new bone layers on the cortical surface. This phenomenon called Adaptive Periostite Ossificante can change the morphology and relationship between contiguous structures as well as function. The periosteum reaction explains the mechanism of action, limitations and efficiency of these procedures whenever wisely indicated and biologically aware.
