The purpose of this investigation was to comparatively evaluate the soft tissue cephalometric changes concurrent to the Class II, division 1 malocclusions treatment with Activator-headgear appliance. Twenty-four individuals formed the activator-headgear group, while others twenty-four, presenting the same malocclusion, that didn’t receive any intervention, served as controls. The lateral headfilms were taken at the beginning and at the end of the observation period and, with the computerized cephalometrics aid, they were digitalized, the cephalometric analyses done and the results submitted to the statistical tests. According to the methodology employed, our findings suggested that the appliance increases mandibular plane inclination, didn’t modify the posterior inferior height and the sagital and vertical position of the upper lip. The lower lip and the soft menton were very slightly modified by the orthopedic appliance, but the mentolabial sulcus showed a significantly depth decrease, compared with the control group. The anterior inferior hard and soft facial heights and the lower lip height increased more in the treated groups and this was of statistical significance too.
