Although firstly reported in 1938, the surgically assisted rapid palatal expansion (SARPE) was only recognized at the last century finale, in response to the progress achieved in orthognathic surgery, along with the patients’ interest towards orthodontics esthetic results. In order to provide a basic background on this technique, the authors briefly reviewed the literature, looking forward to attending professionals willing to overcome their pitfalls in private practice. Nevertheless, one could not overlook some details of the surgical technique, which the orthodontist-surgeon interaction depends from. In this context, a classic case of SARPE is reported to illustrate this technique feasibility, as well as its ideal protocol to attain both short and long-term successful outcomes.
